Drop-off forms are available on site at Rick's Performance, but if you would like to fill out the form ahead of time, you may do so using our electronic version (pdf format). You need Adobe Acrobat Reader in order to download and use the form.

How to Use the Online Drop-off Form:Select the image to the right to open the form in Adobe Reader. You can tab through each item of the form and type in the information as you go. Remember to print out a copy after you've filled in the form fields! Adobe Reader will not allow you to save a copy of the pre-filled form.

...OR, if you prefer to do it the old-fashioned way, just print out a copy of the form and fill in the information by hand.

Next Step:leave your car in our parking lot and submit the form in our drop-off box located by the front door of the shop. We'll contact you as quickly as possible (the following business day) to discuss the details of your repair(s)!
